Dietary
supplements containing ephedrine alkaloids pose short-term and
long-term risks. Ephedrine alkaloids are members of a large family
of sympathomimetic compounds. These compounds increase blood
pressure and heart rate.
Such
sympathomimetic effects raise three concerns.
they
can induce arrhythmias in susceptible people, such as those with
underlying coronary artery disease;
increased
mortality has been observed in patients with congestive heart
failure who were treated with sympathomimetic drugs; studies that
showed these adverse effects occurred in about 3 months of product
use;
elevated
blood pressure.
Although
the pro-arrhythmic effects of dietary supplements containing
ephedrine alkaloids typically occur only in susceptiable
individuals, the long-term risks from elevated blood pressure can
occur even in nonsusceptible, healthy individuals.

The
aspirin boom is a twentieth century phenomenon but the origins of
aspirin go back thousands of years. Certain shrubs, plants, and
trees contain a substance known as salicin (from the Latin "salix"
for willow).
The
Greeks and the Romans were well aware of the pain- killing
properties of the willow tree. They chewed willow leaves and
concocted willow bark brews for use as a general analgesic. It
wasn't until the mid-nineteenth century though that salicylates were
combined with another chemical structure from the acetyl group
(acetic acid is the chief acid of vinegar) to form acetylsalicylic
acid or aspirin.
In
the 1890s two German chemists finally discovered a method for cheap
synthetic production of acetylsalicylic acid, and the basis for a
new industry was born.
It
has become not only humanity's favorite drug for headaches and
muscular pains but the most popular drug in the world. The US
produces some 30 tons of aspirin a day and consumes 50 billion
tablets a year, says Dr. Neil Wigder, author of Over-the-Counter
Drugs. That's an average of 200-300 tabs a year for every man, woman
and child.
Aspirin's
main competition as an OTC headache remedy in America is
acetaminophen, but unlike aspirin it does not reduce
inflammation though does not upset the stomach or cause
gastrointestinal bleeding as aspirin does.
Aspirin's
side effects may be quite serious. Apart from those two mentioned
above, allergic reactions, especially if you have asthma, are not
uncommon.
17
Constant
use of aspirin can interfere with retention of vitamin С
or cause kidney disease, chronic ulcers or anemia.
But
all these side effects overweigh positive role of aspirin which
works in the brain (to improve cerebral circulation, and prevent
thrombosis), in the heart (to prevent vascular occlusion and
clotting) and locally at the site of the injury or pain. It has a
number of physical effects on body chemistry (e.g. it steps up the
body's consumption of oxygen), its analgesic effects are well known.
Only in 1971 the mechanism of its action was decoded by John Wein:
it blocks the action of the body's prostaglandins, hormone-like
substances that play a role in cell metabolism. This discovery
brought John Wien his Nobel prize.
Aspirin
is mostly manufactured in tablets, for the uses in cardiology it's
soluble forms are preferable. It can be given as a rectal
suppository if vomiting prevents taking medication by mouth.
Suppositories, though, irritate the rectum if used repeatedly
so their use should be limited. These suppositories require a
prescription. Aspirin can be purchased over the counter, but should
be used properly, all the more so if a patient has a past history
of ulcer or any gastrointestinal problem.
Aspirin
usually has a shelf-life of about three years but sometimes shorter
periods are quoted. In America which is believed to be the biggest
consumer of aspirin, people usually buy big jars of the cheapest
U.S.P. aspirin at the cut-rate drugstore. If the bottle contains
vinegary odor when opened, the aspirin has begun to deteriorate and
should be discarded.
As
you remember from the previous course U.S.P. stands for "United
States Pharmacopeia". While not an absolute guarantee that
the drug is the best, it does mean, that the drug has met certain
standards in composition and physical characteristics. The same is
true of the designation N.F. which stands for "National
Formulary".
